Purpose |
The transition services helps young people move from Children Social services to Adult social care services with the assistance of a Transition Co-ordinator. The Transition Co-ordinator helps facilitate this change by helping young adults plan and make choices about what they want to do when they leave school. The purpose of the survey was to gather the views and opinions of these young adults, with learning disabilities, regarding the transition process. |
